How do I request a new phone or request a phone be reassigned?

These requests must come from a full-time faculty/staff employee, if you are a student employee or intern, please have your supervisor submit this request.


If need to request the acquisition of a new physical phone for your department, please provide the following information:

  • ACU email of the individual receiving the phone.
    • If you are not requesting for an individual, but for a department, please specify which department and provide contact for the supervisor of the department if you are not the supervisor.
  • Building and room number of the expected physical location of the phone.
  • A FOAP for the purchase of the phone.
    • The base model is $200, while the model with a touchscreen is $250.

 

If need to request a Zoom phone be assigned, please provide the following information:

  • Whether you are requesting a new extension/number or reassignment of an existing extension/number.
    • If a reassignment, the extension/number you are wanting reassigned.
  • The ACU email of the recipient of the Zoom phone extension/number.
